
Cardiovascular Interventions explores the reliability of angiography-derived computational physiology in assessing coronary circulation during transcatheter aortic valve implantation (TAVI) for severe aortic stenosis. The study examined 198 stenotic coronary arteries in 123 patients and compared angiography-based metrics to wire-based measurements. The findings reveal that Murray's law-based quantitative flow ratio is as effective as fractional flow reserve (FFR) in identifying ischemia-causing stenoses, outperforming instantaneous wave-free ratio. The study demonstrates that angiography-derived physiology offers a dependable and systematic assessment of coronary circulation in complex scenarios, shedding light on a potential alternative to pressure-wire assessments.
