FH is a genetic condition that leads to high cholesterol levels and an increased risk of heart disease. Worldwide, 35 million people have FH, but most remain undiagnosed or undertreated. Current FH care is guided by a useful and diverse group of evidence-based guidelines, with some primarily directed at cholesterol management and some that are country-specific. The guidance aims to improve FH care by integrating clinical practices and implementation strategies, benefiting millions of undiagnosed or undertreated patients globally.

Evidence indicates that ASC speck formation triggers caspase-1 activation, gasdermin D-mediated pyroptosis, and IL-1β/IL-18 signalling, creating self-amplifying inflammation and tissue breakdown. The model could refine diagnosis and help redefine the biological limits of vital pulp therapy.

Cyclosporine 0.09% and 0.1% ranked highest for corneal staining improvement, while tacrolimus showed early improvements in tear production and stability. Varenicline increased tear production but needs further comparative validation.
