Involvement of the secosteroid vitamin D in autoimmune rheumatic diseases and COVID-19

This review discusses the role of vitamin D, specifically its active form 1,25-dihydroxy vitamin D3, in modulating immune responses. Vitamin D can downregulate inflammatory responses, support the adaptive immune system, and modulate the innate immune system's response to invading pathogens. Low serum concentrations of vitamin D have been linked to autoimmune rheumatic diseases, and vitamin D supplementation has been shown to improve prognosis and reduce their incidence. Additionally, 1,25-dihydroxy vitamin D3 has downregulated both the early viral and later cytokine-mediated hyperinflammatory phases of COVID-19. The review emphasizes the importance of monitoring serum 25(OH)D3 concentrations and appropriate supplementation following clinical trial-based approaches.
