The study evaluates the biological role of pleural fluid plasminogen activator inhibitor (PAI)-1 and sonographic septations in pleural infection. The study showed that the pleural fluid PAI-1 concentration is significantly associated with the presence and severity of sonographic septation, length of hospital stay, and 12-month mortality in patients with pleural infection. The study highlights the importance of maintaining the balance between the activity of the plasminogen activator and PAI-1 in the pleural cavity to prevent morbid complications. The uPA/PAI-1 ratio may predict sonographic septation status, septation severity, the extent of hospitalization, and mortality of patients with pleural infection. The study suggests that further investigation of the uPA/PAI-1 ratio may lead to the identification of another novel biomarker of sonographic septations and reinforce the detrimental consequence of excessive inhibition of fibrinolysis by PAI-1 in pleural infection.
