NEPHROLOGY

In a randomized, double-masked, noninferiority phase 3 trial, the efficacy and safety of letermovir were compared to valganciclovir for the prevention of cytomegalovirus (CMV) disease in CMV-seronegative kidney transplant recipients who received an organ from a CMV-seropositive donor. The study involved 589 participants who were administered either letermovir or valganciclovir for up to 200 days after transplantation. The primary outcome, CMV disease through posttransplant week 52, was observed in 10.4% of letermovir recipients compared to 11.8% of valganciclovir recipients, demonstrating noninferiority. Letermovir also showed lower rates of leukopenia or neutropenia and fewer discontinuations due to adverse events. These findings support the use of letermovir as an alternative prophylactic treatment for CMV in this patient population.
