Liberal vs. Restrictive Blood Transfusion in MI and Anemia

Liberal blood transfusion is safer for MI patients with anemia. In the MINT trial (3,504 patients), those with baseline heart failure (HF) had worse outcomes, with a 30-day death or HF rate of 18.0% vs. 10.0% in those without HF. A restrictive transfusion strategy led to numerically worse outcomes, especially in HF patients. These findings support a liberal transfusion approach for MI patients with anemia, particularly those with HF.
