
A study compared the effects of two types of tracheostomy heat and moisture exchangers (HMEs) on the health of the tracheobronchial mucosa, oxygenation, humidification, and patient preference. The nasal passage and upper airways normally humidify, warm, and filter inhaled air, but tracheostomy tubes bypass these processes. The study found that both HMEs improved mucosal inflammation and reduced mucus production. However, the Shikani Oxygen HME™ (S‐O2HME), which uses a ball type and turbulent airflow, showed greater improvements compared to the Mallinckrodt Tracheolife II DAR HME (M‐O2HME), which uses a flapper type and linear airflow. The S‐O2HME also improved oxygen saturation (SpO2) more effectively at all oxygen flow rates. The majority of subjects preferred the S‐O2HME. The study recommends regular HME use to optimize pulmonary health in tracheostomy patients, and the use of newer ball-type speaking valve technology along with HMEs.
