Longitudinal immune monitoring of patients with resectable esophageal adenocarcinoma treated with Neoadjuvant PD-L1 checkpoint inhibition

A study analyzed immune responses in 50 patients treated with neoadjuvant therapy and immunotherapy (atezolizumab). Non-pCR patients exhibited an immunosuppressive phenotype, characterized by increased regulatory T cells and monocytes. These subsets correlated with gene expression signatures associated with tumor aggressiveness. After surgery, certain immune markers were linked to early recurrence. Findings suggest challenges in using immune checkpoint inhibition and the importance of refining treatment approaches.
