Despite low prevalence, HDV infection significantly increases the risk of liver fibrosis in HBV patients. Among 914 participants, 1.4% tested positive for HDV, and 61.5% showed detectable HDV RNA. Key factors associated with liver fibrosis included HDVAb positivity (aOR 11.7), male sex (aOR 5.4), and elevated ALT levels (aOR 4.4). Findings highlight the urgent need for improved HDV screening and management to prevent cirrhosis.
