The study aimed to explore the relationship between dietary potassium and sodium intake with incident cardiovascular disease (CVD) among Japanese patients with type 2 diabetes (T2DM). Part of the JDCP study, this nationwide prospective analysis included 1,477 individuals aged 40–75 who completed a Diet History Questionnaire at baseline. Over a median follow-up of 7 years, no significant associations were found between potassium intake and CVD incidence. However, when stratifying hazard ratios (HRs) by potassium and sodium intake tertiles, low potassium intake coupled with high sodium intake significantly elevated the HR for CVD (2.79 [1.02–7.63] for the lowest potassium and 3.92 [1.30–11.79] for higher sodium).
