A cohort study of 416,825 prostate cancer patients from the SEER database (2010–2021) reveals that low PSA levels (≤4.0 ng/mL) are associated with increased all-cause and prostate cancer-specific mortality compared to PSA levels of 4.1–10.0 ng/mL. The risks are most pronounced within the first 5 years post-diagnosis. These findings challenge current assumptions about low PSA levels and highlight the need for improved diagnostic and therapeutic strategies.
