Lower Extremity Strains in the US National Football League, 2015-2019

This study examined the incidence of lower extremity (LEX) strains, such as hamstring, quadriceps, adductor, and calf strains, in professional American football. Over five years, 5,780 LEX strains were reported among 2,769 players, with a one-year risk of 26.7%. The majority of LEX strains were hamstring injuries (54.7%), followed by adductor (24.1%), calf (12.6%), quadriceps (8.3%), and multiple muscle groups (0.3%). Preseason practices and regular season games accounted for the most reported injuries. Defensive secondary players had the highest proportion of time-loss LEX strains. LEX strains resulted in an estimated 16,748 missed participation days per year. Future injury reduction efforts should focus on safe returns during training camp and reducing injuries during regular season games.
