The study investigated the association between glucagon-like peptide-1 receptor agonists (GLP-1RAs) and long-acting insulins (LAIs) in type 2 diabetes patients regarding cirrhosis and hepatocellular carcinoma (HCC) risks. Using Taiwanese cohort data, 7171 propensity score-matched pairs were analyzed. GLP-1RA use showed a significant reduction in composite liver disease risk (HR: 0.56), cirrhosis (0.59), and HCC (0.47) compared to LAIs. Robustness was confirmed through sensitivity analyses. The findings suggest that, among injectable glucose-lowering agents, GLP-1RAs may be associated with lower risks of cirrhosis and HCC in type 2 diabetes patients.
