Mandibular Osteoradionecrosis: Defining the Microbial Milieu and Antimicrobial Resistance at the Time of Rescue Flap Surgery

A recent study examined patients with mandibular osteoradionecrosis (ORN) who underwent anterolateral thigh fascia lata (ALTFL) rescue flaps between 2011 and 2022. Results showed 57.7% had bacterial species, 34.6% had fungal species, and 26.9% had multiple bacterial species. Gram-positive cocci were mostly sensitive to antibiotics, but a resistant Staphylococcus aureus case was found. Candida species caused all fungal growth. Some cases exhibited multidrug-resistant gram-negative bacilli. The findings highlight the importance of tailored antibiotic therapy based on cultures.
