This study examined outcome predictors in acute ischemic stroke (AIS) patients aged 16 to 55 years, focusing on those under 45 and those between 45-55 years, who underwent endovascular treatment (EVT). Of the 1255 patients analyzed, 34.8% in the older group and 25.6% in the younger group had poor 90-day functional outcomes. For older patients, poor outcomes were associated with baseline stroke severity, diabetes, poor collateral blood flow, and slower recanalization time. Among younger patients, poor outcomes correlated with stroke severity and delayed recanalization, while better outcomes were linked to initial scan results and successful recanalization. Fast and successful recanalization had a pivotal role in determining clinical outcomes.
