Mechanisms and management of loss of response to anti-TNF therapy for patients with Crohn's disease

In a study analyzing the effectiveness of infliximab and adalimumab for treating Crohn's disease, researchers found that only about a third of patients achieved remission after 3 years of treatment. Patients with low drug concentrations during the induction period were more likely to experience treatment failure by year 3, suggesting that higher drug levels early in treatment could improve long-term outcomes. Notably, development of anti-drug antibodies leading to undetectable drug concentrations was more common with infliximab, particularly in patients carrying the HLA-DQA1*05 risk variant. However, concomitant use of immunomodulators reduced the risk of developing these antibodies for both drugs. The findings underscore the importance of optimizing drug concentrations and considering immunomodulator use to enhance treatment efficacy in Crohn's disease.
