The meta-analysis on pulse pressure variation (PPV) and stroke volume variation (SVV) as predictors of fluid responsiveness drew our attention with interest and skepticism. The study explored these variables in various patient subgroups but failed to assess their performance in conditions conducive to their reliable use, potentially impacting the reported moderate predictive value. When physiologic limitations are respected, both PPV and SVV serve as reliable predictors, as extensively discussed in prior literature. Additionally, the practical advantage of not requiring cardiac output monitoring should be highlighted, particularly for PPV monitoring.
