A study was conducted to compare the efficacy, safety, and tolerability of injectable incretin-based glucose-lowering medications (IBGLMs) to basal insulin treatment in type 2 diabetes patients. The results showed that IBGLMs reduced HbA1c levels by 0.48% more than basal insulin treatment, with long-acting GLP-1RAs and tirzepatide being the most effective. In addition, all IBGLM subgroups resulted in lower body weight and reduced hypoglycemia and blood pressure while improving lipid variables. However, IBGLM treatment was also associated with more nausea, vomiting, diarrhea, and higher discontinuation rates. The study concludes that highly effective IBGLMs are superior to basal insulin treatment and should be considered the first choice of therapy for most type 2 diabetes patients.
