The present study aimed to compare different types of metabolic surgery with non-surgical therapy for the treatment of type 2 diabetes (T2D). Thirty-six randomized clinical trials were included, comparing various surgical procedures such as gastric banding, gastric bypass, sleeve gastrectomy, and others, with medical therapy. Metabolic bariatric surgery (MBS) showed superior outcomes in terms of reducing HbA1c, achieving diabetes remission, and reducing body mass index (BMI) compared to non-surgical therapy. One-anastomosis gastric bypass and sleeve gastrectomy were particularly effective in reducing HbA1c. All surgical procedures except for greater curvature plication resulted in increased diabetes remission rates compared to non-surgical therapy.
