Micrografts Show Promise in Maxillary Ridge Augmentation

This prospective controlled study evaluated the regenerative potential of micrografts from periosteum and bone tissue combined with an anorganic xenograft for vertical ridge augmentation in the posterior maxilla. Twenty-four sites in 19 patients were treated using the Barbell Technique®. The Control Group (CG) received a xenograft, while Test Group 1 (TG1) and Test Group 2 (TG2) received xenografts with periosteal and bone micrografts, respectively. Histomorphometric analysis revealed no significant differences in vital mineralized tissue (VMT) for inlay grafts, but onlay grafts showed CG had higher VMT compared to TG2. Immunohistochemistry indicated slightly higher VEGF levels in CG and TG1. All groups demonstrated similar bone gain on CBCT. The study suggests periosteal micrografts may enhance bone formation in onlay reconstructions, while bone-derived micrografts showed limited effect
