Mineralocorticoid receptor antagonist use and the effects of empagliflozin on clinical outcomes in acute HF

This study, based on the EMPULSE trial (NCT04157751), investigates the efficacy and safety of empagliflozin in acute heart failure (AHF) patients, considering baseline mineralocorticoid receptor antagonist (MRA) use. The primary outcome, encompassing all-cause death, heart failure events, and changes in Kansas City Cardiomyopathy Questionnaire (KCCQ) scores, demonstrated empagliflozin's benefit regardless of baseline MRA use. The analysis supports the early use of empagliflozin alongside MRA therapy in AHF patients, emphasizing its clinical advantages and tolerability.
