The concept of patient-self-inflicted lung injury (P-SILI) is proposed as strenuous spontaneous breathing can also be harmful, similar to ventilator-induced lung injury (VILI). However, establishing P-SILI as a pathological entity is challenging, and few P-SILI models exist.

Evidence indicates that ASC speck formation triggers caspase-1 activation, gasdermin D-mediated pyroptosis, and IL-1β/IL-18 signalling, creating self-amplifying inflammation and tissue breakdown. The model could refine diagnosis and help redefine the biological limits of vital pulp therapy.

Varenicline increased tear production but needs further comparative validation.
