A randomized trial at a tertiary referral center compared a novel non-ablative Nd:YAG/Er:YAG dual laser treatment with topical steroid therapy for vulvar lichen sclerosus (LS). In a 2:1 randomization, 44 women received laser treatment, while 22 received steroid applications. The laser group showed a significantly greater decrease in LS score (-2.34 vs. -0.95, p < 0.001) and higher patient satisfaction (p = 0.035) at the 6-month follow-up. Laser therapy demonstrated safety, tolerability, and potential as an alternative to corticosteroids, but regular follow-ups are advised due to LS's premalignant nature.
