New CDC Classifier Tracks Origins of Drug-Resistant Malaria

The study details the development and application of a bi-allele likelihood (BALK) classifier by the CDC to trace the geographic origins of drug-resistant Plasmodium falciparum in U.S. travel-related malaria cases. Utilizing sequences from Pfs47 and Pfcpmp, the classifier was trained on published genomes of known origin. Analysis of 380 CDC-received blood samples (2018–2021) with reported travel histories (n=234) achieved a 96% concordance between classifier-predicted origins and reported travel data. In two incongruent cases, the classifier predicted African origin for a Costa Rican-traveled specimen and Asian origin for one linked to Sierra Leone. Applied to 135 samples without travel histories, the classifier supports CDC’s Malaria Resistance Surveillance (MaRS) by providing geographic context for resistance markers, facilitating improved tracking of resistant malaria entering the U.
