This review discussed the current state of cancer biomarkers for the clinical management of pleural mesothelioma, a highly aggressive cancer linked to asbestos exposure. Diagnosing mesothelioma can be challenging, and treatment response is often short-lived despite the use of immune checkpoint inhibitors. The loss or alteration of specific tumor suppressor genes, such as CDKN2A and BAP1, is common in mesothelioma and can serve as valuable tissue-based biomarkers for diagnosis. However, progress in identifying soluble biomarkers has been limited, with mesothelin remaining the standard. Studies exploring predictive biomarkers for immune checkpoint inhibitor response have been disappointing so far. Identifying biomarkers for mesothelioma remains a crucial research area to enhance patient outcomes in this deadly disease.
