
A retrospective cohort study identified key risk factors for the development of HELLP syndrome in patients with pre-eclampsia (PE) with severe features and developed a predictive scoring model. Among 445 patients analyzed, 69 had HELLP syndrome, while 376 did not. Independent risk factors included maternal age <40 (OR=2.28), nulliparity (OR=2.22), mild hypertension (OR=2.31), epigastric pain (OR=3.41), and placental abruption (OR=6.38). A predictive model demonstrated reliable performance with an area under the curve (AUC) of 0.765, indicating potential clinical utility. These findings provide a framework to identify high-risk patients and improve management strategies for HELLP syndrome in the context of severe PE.
