A review of preoperative functional capacity assessment tools reveals major evidence gaps, particularly for patients with obesity, arthritis, and disabilities. While cardiopulmonary exercise testing (CPET) was the most studied (132 studies, 32,662 patients) and showed predictive validity for cardiorespiratory complications, most tools lacked data on reliability, feasibility, and correlation with standards. The findings highlight a need for further research into inclusive and reliable assessments to enhance surgical risk evaluation.
