
A nationwide analysis of beta-blocker prescriptions in China from 2018 to 2023 revealed metoprolol as the most prescribed beta-blocker, constituting 62.2% of total prescriptions. Elderly patients (65.7%) and males (57.0%) were the primary recipients. Off-label uses were prevalent, including sotalol for hypertension (18.25%) and esmolol for angina (12.94%) and heart failure (14.98%). Adverse drug events analysis identified novel risks such as BRASH syndrome (metoprolol: ROR = 391.285; carvedilol: ROR = 256.459), acute kidney injury (bisoprolol: ROR = 5.641), and sleep disorders (propranolol: ROR = 10.98). The study underscores the need for cautious beta-blocker use and monitoring of emerging adverse events to ensure safer, personalized treatments in clinical practice.
