The study developed the obstructive sleep apnea morphologic scale (OSAMS) as an alternative screening tool to identify severe obstructive sleep apnea (OSA) in patients awaiting polysomnography (PSG). The study included 149 patients presenting with sleep apnea symptoms and created OSAMS using five parameters: three internal (tonsil size, oropharyngeal passage, and hypopharyngeal-laryngeal examination) and two external (neck circumference and body mass index). A moderate positive correlation was identified between OSAMS scores and the apnea-hypopnea index (AHI) from PSG (rho = .491; P < .001). A total score greater than 4 yielded 80.3% sensitivity and 53.9% specificity for detecting severe OSA. OSAMS demonstrated superior efficacy compared to existing questionnaires, highlighting its potential as a practical screening approach based on morphological assessments during physical examination
