Results from a study of 215 patients who underwent catheter ablation for atrial fibrillation (AF) revealed that high serum neurofilament light chain (NFL) levels (≥300 pg/ml) independently predicted AF recurrence, with an adjusted hazard ratio of 3.756 (95% CI: 1.392 to 10.136). Over an average follow-up of 10.69 months, 29 patients experienced AF recurrence. NFL levels consistently predicted recurrence across various subgroups and showed a linear relationship throughout its range. Incorporating NFL into the CHA2DS2-VASc score model improved recurrence risk prediction, evidenced by enhanced net reclassification improvement (0.464) and integrated discrimination improvement (0.087). These findings suggest that NFL could be a valuable biomarker for risk assessment and decision-making in AF patients post-ablation
