Novel Insight into Pulmonary Fibrosis and Long COVID

The study evaluates the lack of knowledge regarding the histopathologic manifestations and underlying mechanisms of residual coronavirus disease (COVID-19)–associated lung abnormalities. The study identified several signaling pathways involved in the initiation of pulmonary fibrogenesis in severe COVID-19. The study found evidence of widespread microvascular occlusions due to capillary micro-thrombosis of pulmonary and bronchial vessels and an increasing prevalence of intussusceptive neoangiogenesis and consecutive fibrotic remodeling over the course of hospitalization. The postmortem results showed distinct fibrotic remodeling patterns in post-COVID alterations compared to chronic interstitial lung disease. The findings provide a sound foundation for a better understanding of the underlying pathophysiology of severe COVID-19 and its trajectory toward interstitial fibrosis, which may have implications for long COVID.
