Cyclic vomiting syndrome (CVS) is characterized by episodes of nausea and vomiting, separated by symptom-free intervals. The pathogenesis of CVS is poorly understood. Limited data exist on evaluating impaired gastric accommodation as a mechanistic means for symptoms. We aim to determine if CVS patients demonstrate impaired gastric accommodation applying a nutrient drink test (NDT) protocol. Through this single-blinded pilot clinical trial, patients with CVS per Rome IV critera and healthy controls were assessed for presence of impaired gastric accommodation by administering an established NDT protocol. Statistical analysis was performed, with data presented as medians and interquartile range. To our knowledge, this is the first NDT protocol in CVS evaluating the role of impaired gastric accommodation and hypersensitivity as a possible pathophysiologic mechanism. Findings from this study suggest the presence of gastric hypersensitivity in a subset of CVS patients. These results provide the foundational data necessary for future larger testing of NDT and diagnostic accuracy in CVS.

A Swedish population-based genetic study using data from 1,510,974 individuals identified five interconnected clusters of psychiatric and substance use disorders: internalizing, externalizing, neurodevelopmental, eating, and psychotic disorders. Genetic risks for major depression and drug use disorder were central, while ADHD, bipolar disorder, OCD, and autism spectrum disorder bridged different clusters.

A real-world study of 1,121 patients with rheumatoid arthritis found that published applications of EULAR difficult-to-treat RA criteria identified different patient groups, with prevalence ranging from 5.5% to 11.7%. A two-year threshold from first advanced therapy initiation classified approximately 40% as early and 60% as late D2T-RA, supporting improved temporal stratification.

An umbrella review of 69 systematic reviews covering 970 studies and 221 factors identified six factors with convincing evidence of association with psychotic disorders. High population attributable fractions were observed for childhood adversities (40.69%), Toxoplasma gondii IgG positivity (20.47%), frequent cannabis use (12.75%), and clinical high risk for psychosis (12.68%).

PEDIATRICS

A validation study found that Norwegian versions of the TOPSE and Teen TOPSE questionnaires demonstrated good-to-excellent psychometric properties in parents of children with pediatric acquired brain injury. Internal consistency was excellent (Cronbach’s α=0.94 and 0.96), while test–retest reliability was strong (ICC=0.90 and 0.89).
