
The prevalence of obesity has increased rapidly and is now a major health concern associated with cardiometabolic disorders. This review presents obesity as a new evolutionary condition, summarizes the epidemiological evidence of its detrimental consequences, and discusses the mechanisms involved in the association between obesity and cardiometabolic diseases. The role of potential moderators of this association is also examined, including the "metabolically healthy obesity phenotype," the "fatness but fitness" paradox, and the "obesity paradox." Losing body weight, especially excess visceral adiposity, is necessary to minimize the risk of cardiometabolic diseases, even though maintaining optimal cardiometabolic status is the primary goal in individuals with obesity.
