This case series reviews the published literature on obstetric rectal buttonhole injuries and provides further cases from our institution. Repair techniques of rectal buttonhole tears vary among institutions. Despite this variance, most women suffer no short‐term morbidity following these injuries. This review adds to the current literature with examples of different repair techniques and outcomes.

After adjustment, patients with higher TMT had significantly lower odds of severe disability and lower mortality risk, suggesting TMT may serve as an imaging-derived marker of muscle status and prognosis.

CRITICAL CARE

A systematic review examines ICU-acquired weakness prevalence, risk factors, and outcomes in critically ill children.

A systematic review of 14 studies found a pooled prevalence of 52% for ICU-acquired weakness (ICUAW) among critically ill children, with substantial heterogeneity between studies. Researchers identified 34 personal, disease, and treatment-related factors and 11 associated biomarkers. The findings emphasize early detection, prevention, and standardized diagnostic criteria in pediatric intensive care.

NEUROLOGY

Long sleep duration was associated with increased all-cause mortality risk, potentially mediated partly by systolic blood pressure.

A prospective Taiwanese cohort study of 3,305 participants followed for a median of 30.8 years found that sleeping ≥9 hours was associated with a 17% higher risk of all-cause mortality (adjusted HR 1.17; 95% CI 1.01–1.35). No significant association was observed with cardiovascular mortality. Systolic blood pressure may partially mediate this relationship.

PSYCHIATRY

Experts recommend screening and monitoring for eating disorders before prescribing GLP-1 receptor agonists.

A Swedish population-based genetic study using data from 1,510,974 individuals identified five interconnected clusters of psychiatric and substance use disorders: internalizing, externalizing, neurodevelopmental, eating, and psychotic disorders. Genetic risks for major depression and drug use disorder were central, while ADHD, bipolar disorder, OCD, and autism spectrum disorder bridged different clusters.

RHEUMATOLOGY

Study finds substantial variation in D2T-RA classification and proposes a two-year framework for early and late disease.

Difficult-to-Treat Rheumatoid Arthritis Criteria Vary Across Clinical Applications

A real-world study of 1,121 patients with rheumatoid arthritis found that published applications of EULAR difficult-to-treat RA criteria identified different patient groups, with prevalence ranging from 5.5% to 11.7%. A two-year threshold from first advanced therapy initiation classified approximately 40% as early and 60% as late D2T-RA, supporting improved temporal stratification.

An umbrella review identifies updated risk factors and potential prevention targets for psychotic disorders.

Updated Risk and Protective Factors for Psychotic Disorders

An umbrella review of 69 systematic reviews covering 970 studies and 221 factors identified six factors with convincing evidence of association with psychotic disorders. High population attributable fractions were observed for childhood adversities (40.69%), Toxoplasma gondii IgG positivity (20.47%), frequent cannabis use (12.75%), and clinical high risk for psychosis (12.68%).

PEDIATRICS

Norwegian TOPSE and Teen TOPSE questionnaires demonstrate strong reliability for assessing parenting self-efficacy after pediatric brain injury.

A validation study found that Norwegian versions of the TOPSE and Teen TOPSE questionnaires demonstrated good-to-excellent psychometric properties in parents of children with pediatric acquired brain injury. Internal consistency was excellent (Cronbach’s α=0.94 and 0.96), while test–retest reliability was strong (ICC=0.90 and 0.89).

GASTROENTEROLOGY

T-TAS showed reproducible assessment of thrombus formation in cirrhosis with thrombocytopenia, warranting prospective validation.

The CirTAS study involving 81 patients with cirrhosis and thrombocytopenia found that the Total Thrombus Analysis System (T-TAS) demonstrated high reproducibility, including at platelet counts below 50 × 10⁹/L. Thrombus formation decreased with worsening disease severity and bacterial infection, supporting further validation of T-TAS for assessing bleeding-related risks.

A study of 455 treatment-seeking Dutch veterans found three distinct profiles of moral injury and PTSD: subclinical (20.9%), moderate (47.9%) and high (31.2%). Greater symptom severity was associated with increased comorbid psychopathology and poorer family functioning, highlighting the importance of assessing moral injury in veteran mental healthcare.
