OPTIMAL Titration Algorithm for Anti-IL-5 Therapy in Severe Asthma

In a 52-week study, patients with clinical control on anti-IL-5 treatment were assigned to either continue with standard dosing intervals or follow the OPTIMAL titration algorithm, which adjusted dosing intervals based on disease control. Of the 73 patients enrolled, 37 were randomized to the OPTIMAL arm, with 78% tolerating dose down-titration. Although the OPTIMAL arm experienced a higher trend in exacerbations (32% vs. 17%; p = 0.13), no severe adverse events related to titration were observed. Lung function and symptom scores remained stable and similar across both groups. The OPTIMAL algorithm demonstrated potential for individualized dosing, though its use may need further refinement to balance efficacy and control.
