
This systematic review examined the outcomes of monochorionic diamniotic (MCDA) twin pregnancies with twin-twin transfusion syndrome (TTTS), comparing those complicated by selective fetal growth restriction (sFGR) to those without sFGR. Fetal loss risk was higher in TTTS pregnancies with sFGR, particularly for donor twins. Survival at birth was lower in the sFGR group, but preterm birth rates were similar. Neurological morbidity was higher in TTTS with sFGR, especially for donor twins. However, survival free from neurological impairment showed no significant difference between the groups, suggesting potential implications for counselling and risk assessment before laser surgery.
