The German program for skin cancer screening was established in 2008 with the aim to reduce mortality due to skin cancer. Our objective was comparison of mortality between patients with melanoma who participate in a screening program and those who do not. Patients who underwent screening and had a first diagnosis of melanoma within two years after screening were assigned to the intervention group. Screening participants had significantly better survival. The unadjusted Cox model showed a hazard ratio (HR) of 0.37 (95%CI 0.30‐0.46). After adjusting for named confounders, the effect remained existent (HR 0.62, 95%CI 0.48‐0.80). Patients who had participated in a screening had lower mortality rates with respect to patients who had not undergone a screening.
