The retrospective study assessed the safety and adherence of early oral immunotherapy for peanut allergy (P-EOIT) in a primary care setting. Sixty-nine children under 36 months in New Brunswick, Canada, participated from 2016 to 2020. All reached the maintenance dose of 300 mg of peanut protein over a median of 29 weeks, and 95.7% continued regular peanut protein ingestion after one year. Only one patient required an emergency department visit and epinephrine during the dose escalation phase (1.4%), with no incidents during the first year of maintenance. The findings suggest that P-EOIT in primary care is safe and associated with high patient adherence.
