Pembrolizumab Shows Superior Efficacy as Second-Line Therapy for Advanced Urothelial Carcinoma

The post hoc analysis of the KEYNOTE-045 study demonstrated that pembrolizumab provided prolonged overall survival (OS) and durable responses in patients with advanced urothelial carcinoma (UC) who experienced progression or recurrence after first-line platinum-based chemotherapy. The study included patients who received pembrolizumab 200 mg every three weeks or the investigator's choice of chemotherapy. Across all subgroups, pembrolizumab showed higher median OS, objective response rate (ORR), and duration of response (DOR) compared to chemotherapy, regardless of the response to prior platinum-based chemotherapy. However, patients who had progressive disease (PD) as their best response to initial chemotherapy exhibited the poorest OS outcomes. These findings support using pembrolizumab as a superior second-line treatment for advanced UC.
