This cohort study with 374 participants (96 Alzheimer's disease (AD), 278 frontotemporal lobar degeneration (FTLD)) evaluates plasma biomarkers for distinguishing AD from FTLD. Plasma phosphorylated tau (p‐tau)217 and the NfL/p‐tau217 ratio were found to effectively differentiate AD from FTLD. The three-range threshold approach demonstrated high sensitivity (95%) and specificity (97.5%), significantly reducing the need for invasive testing, offering a non-invasive diagnostic alternative for AD and FTLD differentiation.
