
A prospective cohort study on primary autoimmune cerebellar ataxia (PACA) explored relapse and prognosis factors. Among 68 patients, 35.3% were male, with a median onset age of 42.9 years. Neuronal autoantibodies were found in 50.8% of patients. First-line immunotherapy responsiveness (84.6%) correlated with a favorable prognosis, along with the absence of peripheral neuropathy/radiculopathy. Relapse factors included onset age ≤ 43 years, peripheral neuropathy/radiculopathy presence, and elevated cerebrospinal fluid protein concentration. These findings offer insights for tailored treatment strategies, potentially enhancing outcomes for PACA patients.
