
Femoroacetabular impingement (FAI) causes hip pain, especially in youth. Untreated FAI raises osteoarthritis (OA) risk, leading to early hip replacement. This study compares hip arthroscopy to non-surgical treatment over 5-year follow-ups. Out of 957 patients with FAI, 132 underwent arthroscopy, and 982 were non-surgically treated. Arthroscopy had lower OA progression (26.5% vs. 35.2%) and similar conversion to hip replacement (6.8% vs. 10.5%). Risk factors for failure included male sex, older age, cam morphology, and initial joint changes. Based on the findings hip arthroscopy seems to prevent OA progression in young FAI patients over a 13-year average follow-up.
