This study aimed to assess prostate cancer–specific mortality (PCSM) in men aged 70–74 years who had previously undergone prostate-specific antigen (PSA)-based screening without a prostate cancer diagnosis. The findings suggest that men in this age group who have not been diagnosed with prostate cancer by the age of 74 have a very low risk of dying from prostate cancer by the age of 85. The study recommends considering discontinuing screening in men with PSA <3.0 ng/ml or previous benign prostate biopsies, while those with higher PSA levels and no prior biopsies may consider continued screening if life expectancy exceeds 10 years.
