In a study comparing percutaneous transhepatic cholangiography drainage (PTCD) and endoscopic retrograde cholangiopancreatography (ERCP) for malignant obstructive jaundice (MOJ), PTCD demonstrated superior efficacy. Technical success rates were 97.1% for PTCD and 85.9% for ERCP in palliative drainage. PTCD showed greater total bilirubin (TB) reduction (53.0 vs. 36.8, p < 0.001) and faster 50% TB reduction (7.1 days vs. 10.5 days, p = 0.017) in the preoperative setting. While PTCD had higher drainage tube displacement (11.8%), ERCP was linked to more biliary infections (22.8%). Overall, PTCD may offer improved liver function outcomes for MOJ patients.
