The study developed and validated a radiomics nomogram combining clinical factors and whole-lung CT radiomics features to assess COPD severity. Involving 1,099 patients across training and validation sets, the nomogram outperformed clinical models in stratifying COPD severity (GOLD I–II vs. GOLD III-IV). The model, which included age, height, and Radscore, achieved AUCs of 0.865, 0.851, and 0.781 in three sets. The radiomics nomogram demonstrated superior clinical usefulness, highlighting CT’s added value in evaluating COPD severity and lung function.
