
The retrospective study of 392 patients with vocal fold leukoplakia assessed malignancy rates based on dysplasia grades. Malignant transformation occurred in 6% of moderate dysplasia cases, comparable to severe dysplasia and CIS (43–55%). The findings validate the 2017 WHO reclassification, emphasizing that moderate dysplasia should be managed similarly to high-grade lesions, requiring close histological monitoring and surgical intervention.
