Reliability of Magnetic Resonance Imaging Criteria for the Preoperative Assessment of Rotator Cuff Tears: A Systematic Review

A systematic review of 75 studies assessed the reliability of magnetic resonance (MR) classifications for preoperative evaluation of rotator cuff tears. Eight categories of outcomes were identified, and the reliability of each category varied from poor to excellent agreement. MR imaging was found to be reliable for detecting full-thickness tears, measuring tear size and morphology, and identifying muscle atrophy using the tangent sign or Thomazeau classification. However, other classifications and outcomes did not demonstrate acceptable reliability. The methodological quality of the reliability studies varied, with some articles showing high quality. Therefore, caution should be exercised when using certain classifications for preoperative assessment of rotator cuff tears.
