
Ischaemic stroke remains a leading global cause of death and disability, with limited therapeutic options beyond mechanical thrombectomy and tissue plasminogen activator, which has a narrow treatment window. Drug repurposing, such as using metformin, offers a promising strategy to expand treatments for ischaemic stroke. This review consolidates data from preclinical and clinical studies on metformin’s preventive and therapeutic potential, examines conflicting findings, and provides recommendations for future research.
