The cross-sectional analysis of 100 trials (2001–2020) found waste due to non-publication, poor reporting, or design flaws. Only 78% were published; 25 had poor reporting, and 47 had design deficiencies. Larger trials and those led by investigators from North America or Europe had significantly lower waste (p < .001), highlighting the need for improved trial design and transparency.
