Return to Sports After ACL Augmentation With Anterolateral Reconstruction (ALR) Harvesting Gracilis Only Compared With ACL Reconstruction With ALR Harvesting Both Hamstring Tendons

A study compared anterior cruciate ligament repair (ACL-Rp) using gracilis autograft with anterolateral ligament reconstruction (ALR) to conventional ACL reconstruction (ACL-Rc) with ALR. ACL-Rp group had less hamstring strength deficit, better early return to sports (61% vs. 69.7% in Knee Santy Athletic Return to Sport test), and more patients authorized for pivot sports (61% vs. 41%). No significant differences in clinical scores and knee parameters at 31.4 months follow-up, with a slight trend in higher failure rate in ACL-Rp (6.1% vs. 0%). Harvesting only gracilis showed promising early results in return to sports and limited impact on strength and re-rupture rate.
